Adrenal & Hormone Testing
Cortisol, DHEA and sex-hormone tests can be considered for specific clinical questions. Fatigue alone does not establish an adrenal disorder. Adrenal fatigue is not a scientifically established diagnosis, and a commercial saliva panel does not validate that label.
Stress symptoms versus adrenal disease
Poor sleep and prolonged stress deserve attention, but adrenal insufficiency and other endocrine diseases require validated diagnostic pathways. Suspected adrenal insufficiency needs appropriate medical assessment; severe weakness, vomiting, fainting or dehydration should not wait for a wellness consultation.
Why collection context matters
Time of day, medicines, menstrual timing, pregnancy, contraception and menopause can affect hormone results. A single value detached from this context can be misleading. The clinician should explain why blood, urine or saliva sampling is appropriate for the question being asked and what the test cannot conclude.
Questions before ordering
Ask whether the result would change your care, whether confirmatory testing is needed and whether an endocrinologist or gynecologist should be involved. Testing offered by the practice is selected through consultation, not a promise that every person needs every hormone measured.
What different tests can and cannot answer
| Clinical question | Possible starting point | Important limitation |
|---|---|---|
| Is thyroid function abnormal? | TSH, with free T4 as appropriate | Interpretation depends on context, medicines and pregnancy. |
| Is autoimmunity involved? | Selected thyroid antibody tests | Positive antibodies do not establish the degree of hormone deficiency. |
| Is there a structural lump? | Examination and ultrasound, with specialist assessment | Normal hormone tests do not exclude an important nodule. |
| Does a food cause allergy? | Reaction history and appropriately selected allergist testing | A food IgG panel cannot diagnose or exclude allergy. |
| Are stress symptoms an adrenal disease? | Medical history and validated testing when indicated | “Adrenal fatigue” is not an established diagnosis. |
